The article is aimed at forming the main regulatory proposals that are required for the development of the segment of manufacturing and dispensing of medicines in the Russian Federation. The obtained results can contribute to the development of mechanisms for providing medicines to citizens and the introduction into clinical practice of personalized pharmacotherapy based on modern pharmaceutical manufacturing technologies and good practices. Particular emphasis is placed on the development of provisions and procedures for organizing a system of assurance and quality control in the individual manufacture of medicines. The article consolidates the research and practical results obtained by the team of authors in 2023 and reveals the main imperatives in improving federal legislation in the areas of individual manufacturing of medicines, in-pharmacy packaging of registered medicines, and the implementation of processes of dilution (reconstitution) of highly toxic finished forms of medicines.

The main purpose of creating the Register of Critical Obstetric Conditions is to organize monitoring of obstetric patients with life–threatening conditions in all subjects of the V.I. Kulakov NMRC for OG&P of the Ministry of Health of the Russian Federation around the clock. The Register of Critical Obstetric Conditions is based on a number of analytical tools – fixed reports. Large amounts of information in the Register of obstetric “near-miss” cases nationwide required the creation of a situation center for subjects', federal districts' and the country's healthcare organizers. The situation center displays information in the form of widgets, analytical panels and heat maps in two modes – current status and information for the selected period. Users with the roles of healthcare organizers have access to the builder and 6 fixed reports. An obstetric's “near-miss” case can contain up to 164 records, and a large amount of information makes it difficult to analyze it. In order to simplify the case analysis, an obstetric's “near-miss” case route has been created. The route is built retrospectively and displays basic information about the dynamics of the patient's condition, location, the methods of intensive therapy, telemedicine consultations, critical deviations in vital signs and laboratory results. Control over the maintenance of the Register obstetrics’ “near-miss” cases is carried out by various levels healthcare organizers through the Registry situation center and by specialists of the National Medical Research Center for Anesthesiology and Resuscitators (for pregnant women) of the V.I. Kulakov NMRC for OG&P of the Ministry of Health of the Russian Federation during audits. The article also reflects the prospects for the development of the analytical block of the Register of Critical Obstetric Conditions.

The article presents the results of the development of new local hemostatic agents based on chitosan and adhesive polymers. By combining chitosan with ingredients bearing guanidine and polyhydrofenol groups, an effective topical hemostatic agent with increased adhesion (including to a wet wound surface) was obtained, which does not require additional materials to fix it.

In addition, a prototype of an agent for local hemostasis based on a polymer composition in the form of a film (xerogel), which, due to its hydrophilicity, has strong adhesion to wet surfaces, has been created.

Both films obtained by the drying method are biodegradable, have good hemostatic properties, and strong adhesion to the wound surface and elasticity. The developed local hemostatic agents are universal and suitable for both external wounds and intrasurgical use when working with parenchymal organs and vessels.

In the article, on the example of the Central, North Caucasus, Volga Federal Districts, topical issues and features of the routing program for patients with malignant neoplasms are considered. The private issues of patient routing in the USA and the UK are highlighted. The results of the analysis of indicators of one-year mortality in the framework of the federal project “Fight against oncological diseases“ are demonstrated.
